Admission Requirements

A. TESTING

For Grade 7 and Transferees

• Duly filled-out Application for Admission Form

• Birth Certificate-NSO Copy (D.M. No. 378, s.2000)

• Photocopy of Baptismal Certificate (for Catholics only)

• Recent Two (2) colored 1x1 ID picture

• Photocopy of Current Report Card

• P 200.00 Testing Fee

• Pencil with eraser (Mongol 2)

• Interview with the Principal (Applicants must come together with his/her parent/guardian and bring the Report Card from previous school attended and filled-out application for admission)

• Recommendation of Principal/Teacher/Guidance Counselor from the previous school attended

B. ENROLMENT

• Original copy of Report Card (Form 138)

• Photocopied Birth Certificate – NSO Copy (D.M. No. 378, s. 2000)

• Certificate of Good Moral Character

• Satisfactory Rating of Admission Test

• Medical Certificate (for special cases if necessary)

• 1pc. Long brown envelope

• Summerians (SMAJ students who took summer classes) shall submit summer grade/s upon enrolment. They shall be under academic probation.

C. ADDITIONAL REQUIREMENTS

For Foreign Students: (AAFS No. AFF-04-089)

1. Basic Requirements

2. Alien Certificate of Recognition (ARC)

3. Special Study Permit (SSP) from the Bureau of Immigration

4. Passport (Present original and photocopy)

5. Can communicate in English and pass the English Language Assessment

For Local Students

1. No failing grade in academic subjects and conduct grade 80% and above.

2. Qualifying Mark/ Pass the SMAJ entrance examination and interviews.

Provisional Enrolment. “The provisional enrolment of the students shall be conditioned on the admission of his valid admission or transfer credentials on or before the last day of the school year.”

• Provisionally enrolled students shall not be enrolled for the next higher level for the next school year, if he cannot submit his valid admission or transfer credentials until the last day of classes; and

• Whatever credits for the subject he may pass shall not be recognized. (Ref: Articile IV. Sec. 129-2010 Revised Manual of Regulations for Private Schools in the Basic Education)

D. STUDENT WITH FAILURE

Students who failed in subjects equivalent to three (3) units or more are retained in same year level; in case of failure in any subject, the student must take Summer Classes. If the student fails to take summer class, he/she is advised to transfer to another school (ATT).
